Psychoses while under Iboga

So, I didn't think it was possible for one to become psychotic under Iboga. Or rather, I had read of one or two people from the online reports that it had happened, but didn't think it was common. I assumed the visions really just happened with eyes closed, like it did for me and others I've sat with and heard from. That one never lost the ability to know what was going on in the 'real' world. I worked with a man who was psychotic for 4 days. By that, I mean total separation from this consensus reality. He was seeing things that I could not, paranoid about getting killed, and thought I was trying to kill him. He was combative physically several times. He was also 6'2 and 200#. It was a little scary (I'm 5'3"), but handled okay as one of his family members was around as well. He had a particular history that included being in a war, and having to flee his home country in a rush because of war. Some of this was brought up while he was under. Anyone have any advice on the frequency of psychosis, how to minimize the person's pain and anguish, what to do when psychotic includes combative? Anyone go through this themselves? (Note: all treatments were performed in a country where Iboga is legal) Thanks, friends. T

Please also note I have worked with so many hundreds of psychotic people as a mental health professional. I am comfortable with psychoses, lol, but want to make the Iboga experience the best I can for those I am honored to sit with. Also, if there are things to do particular to Iboga with psychoses, I'm all ears. Any of you ever 'lose it'? If not, why do you think? When rage came up, what was in place that it didn't become violent? Thanks, all y'all.

I wonder if there is a correlation between freaking out on iboga and any extraction that isolates the ibogaine alkloid. Maybe we would find upon inspection that when all the alkoids are kept in tact that this doesnt happen. Maybe there are alkloids present in the TA extracts that buffer or prevent certain outcomes that only happen when the iboaine alkloid is isolated or singled out . if its not broke why fix it ? mahalos - hp

Harvey, good point. I did use 85% ibogaine. The other 15% were the other alkaloids, including ibogaline. But, certainly not TA. I read of one woman who just treated with TA and had someone psychotic for six days. So, I dunno, but something to think hard on. Also, I've heard that people are getting good results with a mix of TA and ibogaine. Like half and half, or 1/3 ibogaine, 2/3 TA.

Damned interesting, Tia. I am glad you're ok. I'm glad he's ok. This is certainly dangerous, and frankly, maybe this was naive, but I had not considered such a problem with this tx. I was so lucid during my experience that it's hard to imagine losing it like that. At least with this particular medicine. I have lost it (but good) on mescaline containing cactus once. I was as scared as a human can be and still survive. I had the paranoia - thinking my friends were trying to hurt me or even kill me. It was absurd. But I didn't think it was a possibility with iboga. What was the guy treated for? Was it perchance methamphetamine? Was he on Rx's? Without getting too personal, but these things would certainly influence any probability of a breakdown. Also, I wouldn't even think about treating someone without having a benzodiazepine readily available. These things are essential in my opinion. Pre-treatment 'interviews' with a prospective 'patient' may reveal a predisposition to anxiety or a propensity to panic. Did he have prior experience with hallucinogens? This too is important I think. Very important thread my friend...thanks for sharing this Tia. Bless you for all your hard work!

The man was treated for GHB. He also said he did meth 'eevry once in a while', he may have underreported. He was up two days detoxing before we met, couldn't sleep, this is a factor, I think, it did look a bit like sleep dep paranoia, the kind you see in tweakers. Scrip was antidepressant (none a week before tx) and xanax, which we had on hand. And used. No prior with hallucinogenics, not much drug experience at all besides scrip for anxiety, GHB (for body building, initially!), and speed.

Hate to be stupid but what is GHB? I only did Ibogain once..for opiate detox.  There was music playing that my ears was not accustom to.  When things got very loud I made a conscience choice to "check-out".  I was not fearful at all but hated that music. I cant say how long I blacked out but I do remember my providers at my bedside checking on me.  I was fine.  I can see how one can get so overstimulated it that it may take time for the brain to "settle"  as this person needed.  Just thinking.. but maybe a small dose of seroquel may have assisted a bit till the HCL was broken down more in his system.   The liver is a funny organ.. sometimes it can just stop processing or slow up for a short time, then go back to work.  THis is proof with folks on digoxin... same dose daily then one day seeing yellow rings, levels toxic.  I have seen daily bld work, same time, same dose and all of a sudden the digoxin goes to toxic levels.  Just thought.

GHB is a degreaser, lol. Here's a site with good front page information: http://www.projectghb.org/what_is_ghb.htm I can imagine the guys liver was having a time of it. He also had gallbladder problems which he neglected to mention to me. He had surgery scheduled the next month! Please hear that I did a thorough intake. I think he didn't want to jeopardize his treatment, so left out stuff he thought I might not want to hear. Laws, chile' we live and we learn. Sis, what you know about gallbladder disease?

DUUUUUUDE. A case of withdrawal from the GHB precursors gamma-butyrolactone and 1,4-butanediol. Schneir AB, Ly BT, Clark RF. Division of Medical Toxicology, Department of Emergency Medicine, University of California San Diego Medical Center, California Poison Control System-San Diego Division, San Diego, California, USA We describe a case of withdrawal from the gamma hydroxybutyric acid (GHB) precursors gamma butyrolactone and 1,4-butanediol. Symptoms included visual hallucinations, tachycardia, tremor, nystagmus, and diaphoresis. Administration of benzodiazepines and phenobarbital successfully treated the withdrawal symptoms. As predicted from the metabolism of gamma butyrolactone and 1,4-butanediol to GHB, the symptoms were nearly identical to those reported from GHB withdrawal. Because GHB is now illegal in the United States, individuals have begun abusing the legal and easier to acquire GHB precursors. More frequent cases of both abuse and withdrawal from these GHB precursors can be expected. This is an abstract from MedLine

Yeah, I read that post and thought, "she stepped into a minefield"... This is an invaluable case - I guess that's the silver lining here. I have never heard of ibogaine being used to facilitate addiction interruption for GHB. We saw a lot of 'G' in the rave scene. A terrible drug. Ugly and very dangerous. I think people underestimate it's addictive propensities. Now to have your report out and in the archives, maybe it will help others who are considering this type of treatment. I am just glad you're okay Tia. I have no doubt of the thoroughness of your initial screening of the people you choose to help - no doubt whatsoever: you just can't tell how people will react sometimes. Man! Great work, lady...

I gotta stress, I'm no hero. This guy and Iboga did the hard part. 10 days after Iboga, he says no cravings, he looks less swollen, he has no recollection of the 4 days. He's doing well, the ibo glow around him. I'm so grateful he feels well, he says now he just needs to live his life. He's going to get another ultrasound of gallbladder presurgery. I'm curious about the results. Wow. I guess I needed to talk about it.

Interesting Tia, never would have thought Iboga being effective against GHB use...rough stuff from what I hear....and possibly dangerous.

Caution' the combo of alks in Iboga are stimulating' it is way easier to monitor someone for psychotic episodes with moderate does Ibogaine' PTSD is dealt with vrey gently' almost homeopathic doses over a very long time and psycho-therapy as a path to recovery' you only cause max distress by attempting to transition someone of whom might have underlying psychological problems' This is also another reason for the test dose and waiting 6 hours plus before attempting to proceed' A test dose of 200-300mg will wear off at the 8-14 hour mark and also no be to distressing if the person reacts badly' If you are using TA then 2 small doses of 150mg each over 6 hours = 300mg alks'  this shall wear out at 16 hours' hence no problems if a reaction occures' Another note' the bad reaction to Iboga is usualy a psychic one' ie the person expereinces great aggitation and feeds this with fear' There is no reason for any person to reach this stage in Iboga therapy' becasue of maticulioius psychological screening and care in administration' bliss! Nobu +

Update: He never went back for his ultasound, as his gallbladder no longer pains him. He did again try GHB, reports it doesn't do what it once did. Reports continued introspection and a connection to living. How does one define a successful tx? Zero substance use? If so, oh my. Then my tx was unsuccessful. Nobu, please tell me more about your direct experience with GHB detox. How much were your clients taking? Was it their only addiction? Re PTSD, there are numerous anecdotes around the web of healing results using dosing other than homeopathic. Lee Albert's site has good info-insight on treating for psychological healing purposes. The myeboga site. Thanks for the responses, all!

What defines a successful treatment? That's the best question I've heard in a long time... Total abstinence? I don't think so, necessarily. In fact I think that's unusual. More often than not - what I've seen and heard is that folks will report that the drugs don't feel good any longer, etc. Introspection and a connection to living, in themselves are worthy enough for success...Most people using this for addiction interruption will relapse at least once. The difference is that knee-jerk reaction is replaced by wide eyed honesty and a conscientiousness that was not present before. The power of choice is returned to us. We can again be human beings. Good post...
