CONFIRMED FATALITYCAUSALITY: ESTABLISHED

Broomfield, Colorado, United States

March 19, 2021

Anthony "Andy" Haman, 54, died on March 19, 2021, following an ibogaine session at a private residence in Broomfield, Colorado. According to the U.S. Department of Justice, Ameen Alai (also known as Adam Powars) distributed ibogaine and supervised its use, then left Haman unattended; when he returned, Haman was unresponsive and subsequently died. Alai pleaded guilty to one count of ibogaine distribution and received a 48-month federal prison sentence, with the court finding at sentencing that the ibogaine he distributed caused the death. According to CBS Colorado's account of the autopsy, the cause of death was ibogaine toxicity.

The incident occurred at Ameen Alai's private residence in Broomfield, with no licensed treatment center involved. Alai was federally indicted in January 2023 for distribution of a controlled substance resulting in death, and the case was prosecuted under federal drug distribution laws. The U.S. Department of Justice announcements did not name the deceased, but CBS Colorado later identified him as Anthony "Andy" Haman. The federal case number is 22-cr-00339-CNS. At the time of Alai's sentencing in September 2025, Colorado was months away from expected state approval of regulated ibogaine use.
