A published case report describes a 25-year-old man who died following reported ibogaine self-administration for heroin dependence.
The historical sources describe cardiac arrest followed by multiorgan failure. The same patient appears in a poison-center presentation and the later paper (Warrick/Baltarowich - do not create a separate card). Use the original case report to verify date and city before assigning either. Do not merge with the 33-year-old O'Connell case (non-fatal).
