iHow this tracker works — methodology, verification standard, and disclaimer
- Every case listed here is backed by at least one cited public source: a news report, coroner’s report, court record, death certificate, academic paper, or official statement.
- Confirmed means an official record (coroner, court, death certificate, or official statement) directly documents the case and its location. Known means the case is reported in credible media but lacks that level of official confirmation.
- Inclusion of a treatment center or country in this tracker does not itself imply fault, negligence, or a confirmed causal link between ibogaine and the death — it reflects only that a reported death occurred in that context, as described by the cited sources. See each case’s Causality field for whether a causal link has actually been established.
- Cases involving unnamed/anonymous individuals are included when the location and date are reported, even without a name.

- This tracker documents individual cases. For the broader clinical picture — cardiac risk mechanisms, QT prolongation, and what medically-supervised screening protocols do differently — see Ibogaine Deaths: Causes, Statistics, and How Medical Settings Prevent Them.
